The cost of this event is $75 for members and $85 for non-members.  Reservations for this event will be accepted until Monday, March 24^th, 2008.  Refunds for this event will not be given after March 17, 2008.

Please make a copy of this liability form for each person requesting a “Meet Me at the Beach” reservation.  Reservations will not be accepted or confirmed unless each person on the reservation request has filled out and signed this Release of Liability Form.

 

Release of Liability

 

            In consideration of the acceptance of my reservation to attend and participate in the activities sponsored by the D.C. Hand Dance Club at the “Meet Me at the Beach” event held at the Clarion Resort Fontainbleau Hotel in Ocean City, Maryland, on March 27-30, 2008, I the undersigned (hereinafter referred to as “releaser”), for myself, my heirs, executors, administrators, and assigns, hereby release, waive, and discharge the D.C. Hand Dance Club, its officers and directors (hereinafter referred to as “releasees”), from all claims for damages for personal injury, death, and loss or damage to property, whether caused by the negligence of the releasees or otherwise, arising out of releasers’ participation in any activity or function sponsored by the D.C. Hand Dance Club at the “Meet Me at the Beach” event.

 

            Releaser further assumes all responsibility for, and all risks of, bodily injury, death, or loss or damage to property that may occur to releaser while releaser is participating in any activity or function sponsored by the D.C. Hand Dance Club at its “Meet Me at the Beach” event.

 

            Releaser further agrees to indemnify releasees and hold them harmless from any loss or damage they may incur resulting from the negligence of the releaser while participating in any activity or function sponsored by the D.C. Hand Dance Club at the “Meet Me at the Beach” event.

 

            The provision of this Release shall be construed according to the law of the State of Maryland and, if any provision herein is held to be invalid, it shall not affect the validity of the remaining provisions.
